It is the mission of Premier House Calls to bring kind, respectful, up to date, individualized Primary Care to patients who choose to receive our services. It is our goal to deliver care in a framework that combines "best practices" with individual goals and preferences. The outcome goal will be reduced hospitalizations, and increased health and functional wellness that supports independent living. Our staff of experienced Nurse Practitioners will assist you from every phone call to every visit.

Primary Care in the privacy of your home
Our Nurse Practitioners deliver up to date Primary Care that is customized to assist you to achieve your personal health goals and maintain your independence. For your convenience, many preventative and diagnostic tests can be ordered to be performed in your home as well.
Prevent hospitalizations
Our goal is to promote health and wellness by regular exams and by appropriate preventative and diagnostic testing. By rapidly responding to health concerns and changes, health problems can be treated early, before hospitalization may be needed. It is our pleasure to work closely with your specialists or home health caregivers to ensure that you get the best care possible.
Transitional Care
After being discharged from the hospital or rehabilitation facility, an early follow up appointment will help ensure a successful transition back in to your home. By reviewing records and performing an examination, we will evaluate your needs and make certain you have the medications, care, services, and equipment you require. Your progress will continue to be monitored frequently to make certain you achieve your goals.
